Your
family’s personal information is kept carefully and safely by this
practice, and we are very mindful of your rights and privacy. It has
always been our practice to carefully communicate with you by phone or
in person in order not to disseminate info that you do not wish shared.
We ask you to personally fill out a form telling us when we may call you
with appointments or medical information, and we ask you to keep this
info up-to-date. Your child’s medical chart does not leave the office
while he/she is an active patient here. When the child has not been seen
for two years, the chart goes into locked storage for up to 22 years (by
Arizona law) and is accessible only to our authorized personnel.
Your child’s visits to the practice are recorded on paper and moved by
licensed courier daily to our business office where they are entered
into a secured computer software system. Depending on your contract with
your insurance company, they may be electronically transmitted to your
insurance company via encrypted systems. No information is sent via the
Internet e-mail. We make every effort to keep your confidential medical
information private and safe. Dr. Goodsite is our privacy officer. If
you ever have concerns about privacy issues, he will be happy to address
them with you. Please call 296-3248 and ask that Dr. Goodsite discuss
privacy concerns with you. In the event that Dr. Goodsite is out of the
office, please leave a call back number and he will return your call as
soon as he is available.
